​ACT Care Foundation is a proud participant in the Brooklyn, Queens, Manhattan, and Staten Island Social Care Networks (SCNs). We offer Health Related Social Needs (HRSN) Screenings, Care Management and Navigation as well as community/federal referrals to all Medicaid Members, and the following services to eligible Enhanced Medicaid Members:​
  • Home/Pest Remediation and Modification Services (Staten Island, Brooklyn, Queens, Manhattan)
  • Asthma Remediation and Modifications (Brooklyn, Queens, Manhattan)
  • Rent/Temporary Housing (Brooklyn, Queens, Manhattan)
  • Medical Respite (Brooklyn, Queens, Staten Island, Manhattan)
  • Community Transitional Supports (Brooklyn, Queens, Manhattan)
  • Medically Tailored Meals (Brooklyn, Queens, Manhattan)
  • Clinically Appropriate Home Delivered Meals (Brooklyn, Queens, Manhattan)
  • Medically Tailored or Nutritionally Appropriate Food Prescriptions (Brooklyn, Queens, Manhattan)
  • Fresh Produce and Non-Perishable Groceries - Pantry Stocking Delivery (Brooklyn, Queens, Manhattan)
  •  Nutrition Counselling and Education (Brooklyn, Queens, Manhattan)
  • Cooking Supplies (Brooklyn, Queens, Manhattan)
  • Transportation related to HRSN service or care management (Brooklyn, Queens, Manhattan)

FOOD FOR FAMILIES

 ACT is working with people in the NYC Community to help address hunger disparities. ACT partnered with Growing Soul, the USDA, New York City Food Relief to distribute food to families in Staten Island and across the 5 boroughs.
